Privacy policy

How All Care Medical handles personal health information.

All Care Medical collects and keeps personal health information so that you can be treated safely. This page explains what we collect, why, and what you can ask us to do with it.

What we collect

Your name and contact details, your Ontario health card number, your medical history and the medications you take, clinical notes from your visits, test and imaging results, referral and specialist correspondence, and a record of appointments.

Why we collect it

  • To assess, diagnose and treat you, and to plan your ongoing care.
  • To bill OHIP for insured services on your behalf.
  • To contact you about appointments, results and follow-up.
  • To meet the record-keeping obligations that apply to physicians in Ontario.

Who we share it with

Only where it is needed for your care or required by law: with laboratories and imaging services when tests are ordered, with specialists when you are referred, with pharmacies for prescriptions, with the Ministry of Health for billing insured services, and with public health where reporting is legally required. We do not sell personal information and we do not use it for advertising.

How it is kept

Records are held in the clinic's electronic medical record, with access limited to the people who need it to do their job. Records are kept for the period required of physicians in Ontario and then securely destroyed.

Your choices

  • You can ask to see your record and to get a copy of it.
  • You can ask us to correct anything that is wrong.
  • You can ask how your information has been used or disclosed.
  • You can withdraw consent for a use that is not required for your care or by law.

This website

This site does not set advertising cookies and does not track you across other sites. The map is embedded from Google Maps and loads as you scroll down to it; Google may set its own cookies at that point. The appointment request and the contact form do not store anything on this site: they open an email in your own mail app, addressed to the clinic, and you decide whether to send it.

Getting and sharing your records

How do I get a copy of my medical records?

Ask at the front desk and they will tell you what the clinic needs in writing, and what to bring when you collect the copy. Under Ontario’s health privacy law (PHIPA) the clinic must answer within 30 days, or tell you in writing why it needs up to 30 more. Copies aren’t covered by OHIP, so there may be a fee, but the law caps it at reasonable cost and you get an estimate first.

How do I get my records sent here from my old doctor?

Ask your old clinic to send a copy of your file here. They will usually ask you to sign a request, keep the original, and may charge you for the copy. A summary of your conditions, medications and recent results is often quicker than the whole file.

Can my husband, wife or grown child pick up my forms or results?

Yes, once you’ve told the clinic who is coming. Ontario’s health privacy law needs your express consent before the clinic gives your health information to anyone outside your health care team, family included. Ask them to bring photo ID.

Can my employer or insurance company see my file?

Not without your consent, and then only what the release you sign covers, such as a completed disability form. Read an insurer’s release before you sign it: many ask for your whole file. The exception is a workplace injury claim, where the law requires the doctor to report to WSIB.
